For ten years, 1971–1981, the Institute onHuman Values in Medicine (IHVM) played a keyrole in the development of Bioethics as afield. We have written this history andanalysis to bring to new generations ofBioethicists information about the developmentof their field within both the humanitiesdisciplines and the health professions. Thepioneers in medical humanities and ethics cametogether with medical professionals in thedecade of the 1960s. By the 1980s Bioethics wasa fully recognized discipline. We show the rolethat IHVM programs played in defining thefield, (...) training faculty and helping schools todevelop programs. We review the beginnings ofthe IHVM in the crucible of social andtechnological change that led to theestablishment of the IHVM's parentorganization, the Society for Health and HumanValues. We then turn to the IHVM programsthrough which Faculty members receivedfellowships to explore new crossovers betweenthe humanities and the health professions. Wehave not only described the Fellows Program asit existed in 1973–1980, but have completed asurvey of the fellows a quarter of a centuryafter they held their fellowships. We describeother IHVM programs designed to facilitate theinitiation and development of new humanitiesprograms, to explore conceptual issues betweenmedicine and five humanities fields, to conductissue driven or educational method conferencesand to advance humanities programs intograduate education through the Directors ofMedical Education. Welfare science has built its foundations on veterinary medicine and thus measures of health. Since bottlenose dolphins (Tursiops truncatus) tend to mask symptoms of poor health, management in captivity would benefit from advanced understanding on the links between health and behavioural parameters, and few studies exist on the topic. In this study, four representative behavioural and health measures were chosen: health status (as qualified by veterinarians), percentage of daily food eaten, occurrences of new rake marks (proxy measure of social (...) activity), and willingness to participate (WtP) in Positive Reinforcement Training sessions as qualitatively measured by their caretakers. These data were collected multiple times a day, every day over the course of a year from a multi-facility, large sample size (ndolphins= 51), allowing powerful analyses of the relationships between measures. First, it was found that dolphins with a higher WtP score also had a significantly better health status, ate a higher percentage of their daily food, and a lower occurrence of new rake marks. In addition, the WtP score was significantly lower up to 3 days before the weekly veterinary diagnosis of a decrease in health state; the percentage of daily food eaten and new rake mark measures did not show any significant change before such a diagnosis. These results suggest that WtP in training sessions is a potential behavioural measure of dolphin welfare, and an indicator of early changes in the dolphins’ health state. We therefore suggest measurement of WtP as a more practical and non-invasive tool to support veterinary care and general management. More work needs to be conducted to elucidate the influence of social behaviour on health, and to identify other potential welfare indicators, but this long-term study has shown that qualitative measures can be both practical and valid when assessing dolphin welfare. How should we think about synthetic biology—about the potential benefits and risks of these applications as well as the very idea of designed, extensively genetically modi­fied organisms? The lead article in this report sets out our thinking, but the article is rounded out with nine commentaries that sometimes expand on and sometimes argue with our perspective. Jonathan Wolff, a philosopher at the University College of London and a member of the Nuffield Council on Bioethics, and Mark Bedau, a philosopher at (...) Reed College and a participant in several projects aimed at de­veloping artificial cells, discuss the potential consequences of synthetic biology and suggest some reasons for a precau­tionary approach to deliberating about the consequences. Jane Calvert, reader in science, technology, and innovation studies at the University of Edinburgh and a member of the Nuffield Council on Bioethics working party on emerging biotechnologies, recommends, however, that deliberation about the governance of emerging technologies focus less on consequences than on aims and purposes.Gigi Gronvall, senior associate at the Center for Health Security in the University of Pittsburgh Medical Center, and David Relman, a microbiologist and immunologist at the Stanford School of Medicine, offer contrasting per­spectives about a question we raise concerning “the eth­ics of knowledge”—the values at stake in generating and disseminating potentially dangerous information. Gaymon Bennett offers some ruminations on the questions we raise about how synthetic biology might change the human re­lationship to nature in morally significant ways. He argues, too, that we can think about the ethics of synthetic biology more effectively by getting “upstream” in another way—by sitting next to the bioengineers doing the work and study­ing their “everyday practice” rather than by paying attention only to what is thought and said about their work. Colleen Grogan, professor at the University of Chicago's School of Social Administration and an expert on democratic partici­patory processes in governance, builds on our comments about the challenge of creating a meaningful public dialog about synthetic biology. Jim Thomas, a researcher with the ETC Group, challenges our view of how the public debate about synthetic biology is trending. And Sarah Carter, a policy analyst with the J. Summary This paper reconceptualizes Thomas Clifford Allbutt's contributions to the making of scientific medicine in late nineteenth-century England. Existing literature on Allbutt usually describes his achievements, such as his design of the pocket thermometer and his advocacy of the use of the ophthalmoscope in general medicine, as independent events; and his work on the development of comparative pathology is largely overlooked. In this paper I focus on this latter aspect. I examine Allbutt's books and addresses and claim (...) that Allbutt argued for the centrality of comparative pathology in the advancement of medical knowledge. He held that diseases should be studied as biological phenomena and that medicine should be made a biological science. He also argued that comparative pathology should be based upon the idea of evolution, and its study should embrace other nineteenth-century sciences including neurology, embryology and bacteriology. Allbutt's writings reveal that his endorsement of comparative pathology, his promotion of the use of the ophthalmoscope and the thermometer in clinical medicine, and his support of the hospital unit system were part of a single programme. All were grounded in his scientific vision of medicine which emphasized a research culture, a stringent nosological attitude and an integration of laboratory sciences and clinical medicine. (shrink)

Provider claims to conscientious objection have generated a great deal of heated debate in recent years. However, the conflicts that arise when providers make claims to the "conscience" are only a subset of the more fundamental challenges that arise in health care practice when patients and providers come into conflict. In this piece, the author provides an account of patient-provider conflict from within the moral tradition of St. Thomas Aquinas. He argues that the practice of health care providers should (...) be understood as a form of practical reasoning and that this practical reasoning must necessarily incorporate both "moral" and "professional" commitments. In order to understand how the practical reasoning of provider should account for the needs and commitments of the patient and vice versa, he explores the account of dependence provided by Alasdair MacIntyre in his book Dependent Rational Animals. MacIntyre argues that St. Thomas’ account of practical reasoning should be extended and adapted to account for the embodied vulnerability of all humans. In light of this insight, providers must view patients not only as the subjects of their moral reflection but also as fellow humans upon whom the provider depends for feedback on the effectiveness and relevance of her practical reasoning. The author argues that this account precludes responsive providers from adopting either moral or professional conclusions on the appropriateness of interventions outside the individual circumstances that arise in particular situations. The adoption of this orientation toward patients will neither eradicate provider-patient conflict nor compel providers to perform interventions to which they object. But this account does require that providers attend meaningfully to the suffering of patients and seek feedback on whether their intervention has effectively addressed that suffering. (shrink)

Discussions concerning whether there is a natural right to health care may occur in various forms, resulting in policy recommendations for how to implement any such right in a given society. But health care policies may be judged by international standards including the UN Universal Declaration of Human Rights. The rights enumerated in the UDHR are grounded in traditions of moral theory, a philosophical analysis of which is necessary in order to adjudicate the value of specific policies designed to enshrine (...) rights such as a right to health care. We begin with an overview of the drafting of the UDHR and highlight the primary influence of natural law theory in validating the rights contained therein. We then provide an explication of natural law theory by reference to the writings of Thomas Aquinas, as well as elucidate the complementary “capabilities approach” of Martha Nussbaum. We conclude that a right to health care ought to be guaranteed by the state. (shrink)

In eitecting this transformation.much credit is due to the work of Norman Kemp Smith. Kemp Smith argued that preoccupation with Hume's empiricist heritage and his scepticism had blinded scholars to the fundamentally constructive intent of his philosophy, and, indeed, that only from the perspective of his positive achievement was it possible to grasp the nature and significance of Humean scepticism. This paper, in keeping with the basic orientation of Kemp Smith, sets out a central feature in the attempt to describe and assess Hume's constructive, nonsceptical point of view: Hume attributes to general rules "a mighty influence on our actions and understanding..." The other type of general rule, the discussion of which largely occupies this paper, functions to correct certain natural propensities which result in erroneous belief or action if permitted to operate unchecked. The same pattern, a natural tendency requiring correction by general rules, is exhibited in all three books of the Treatise, and can be said to represent one of the basic ingredients in Hume's account of human nature and experience. It is somewhat surprising that in a book which patiently discusses almost evcxy theme in the Treatise, Kemp Smith's study contains no discussion at all of rules. This is the more puzzling as Kemp Smith clearly recognized that by reflection the understanding supplies principles which are normative for the interpretation of experience.5 Our aim in this study is to discuss what for Hume these principles are, and, as far as possible, how they fit into the Humean account of human nature. A fuller understanding of the nature and function of such reflective principles is required to spare Kemp Smith's interpretation from a seeming inconsistency. This apparent inconsistency arises because Kemp Smith takes the slavery of reason to the passions as the "key" to his understanding of Hume, but then develops the view that for Hume reason/s a passion, in Hume's words, "a wonderful and unintelligible instinct." A major contribution of Kemp Smith's work on Hume was the insistence that the epistemological doctrines of Book I were essentially related to the ethical theory of Book III. Recent moral philosophy has found Book III to be of considerable intrinsic interest and relevance to current problems. It is now becoming apparent, however, that Hume's ethical theory is intimately bound up with the philosophy (...) of mind in Book II. This new emphasis upon the significance of Book II for Hume's ethics derives from Dr Pall S. Árdal's book Passion and Value in Hume's Treatise. In view of Árdal's influence on continuing study in this field—witness the recent book by Philip Mercer—it becomes important to point out what I take to be a major error in one part of Árdal's analysis of the relationship between Hume's philosophy of mind and his moral theory. In this article I caution that María Lugones's critiques of Kimberlé Crenshaw's intersectional theory posit a dangerous form of epistemic erasure, which underlies Lugones's decolonial methodology. This essay serves as a critical engagement with Lugones's essay “Radical Multiculturalism and Women of Color Feminisms” in order to uncover the decolonial lens within Crenshaw's theory of intersectionality. In her assertion that intersectionality is a “white bourgeois feminism colluding with the oppression of Women of Color,” Lugones precludes any possibility of intersectionality operating as (...) a decolonial method. Although Lugones states that her “decolonial feminism” is for all women of color, it ultimately excludes Black women, particularly with her misconstruing of Crenshaw's articulation of intersectionality that is rooted within the Black American feminist tradition. I explore Lugones's claims by juxtaposing her rendering of intersectionality with Crenshaw's and conclude that Lugones's decolonial theory risks erasing Black women from her framework. (shrink)

Late in a career of more than sixty years, Thomas Burch, an internationally known social demographer, undertook a wide-ranging methodological critique of demography. This open access volume contains a selection of resulting papers, some previously unpublished, some published but not readily accessible [from past meetings of The International Union for the Scientific Study of Population and its research committees, or from other small conferences and seminars]. Rejecting the idea that demography is simply a branch of applied statistics, his work (...) views it as an autonomous and complete scientific discipline. When viewed from the perspective of modern philosophy of science, specifically the semantic or model-based school, demography is a balanced discipline, with a rich body of techniques and data, but also with more and better theories than generally recognized. As demonstrated in this book, some demographic techniques can also be seen as theoretical models, and some substantive/behavioral models, commonly rejected as theory because of inconsistent observations, are now seen as valuable theoretical models, for example demographic transition theory. This book shows how demography can build a strong theoretical edifice on its broad and deep empirical foundation by adoption of the model-based approach to science. But the full-fruits of this approach will require demographers to make greater use of computer modeling [both macro- and micro-simulation], in the statement and manipulation of theoretical ideas, as well as for numerical computation. How much information from experience does a normal adult remember? The “functional information content” of human memory was estimated in several ways. The methods depend on measured rates of input and loss from very long‐ term memory and on analyses of the informational demands of human memory‐based performance. Estimates ranged around 109 bits. It is speculated that the flexible and creative retrieval of facts by humans is a function of a large ratio of “hardware” capacity to functional storage requirements.

A good deal of attention has been recently focused on the presumed advertising excesses of the healthcare industry in its promotion techniques to healthcare professionals, whether through offering gratuities such as gifts, honoraria, or travel support2-6 or through deception. Two basic concerns have been expressed: Does the acceptance of gratuities bias the recipient, tainting his or her responsibilities as the patient's agent? Does acceptance of the gratuity by the healthcare professional contribute to the high cost of healthcare products? The California (...) Society of Hospital Pharmacists was recently asked by its members to formulate a policy for an appropriate relationship between the Society and the healthcare industry, addressing these concerns. In formulating its policy, it became clear that the Society depended on healthcare industry support, gathered through journal advertising, fees for booths at its various educational events, and grants for speakers. During the last thirty years both the social sciences and humanities in many countries have experienced a huge increase in student numbers, often directly related to national policies aimed at enlarging access to higher education for the majority of a generation. Although this evolution seems globally positive, it has also caused some specific problems within those disciplines, such as anthropology, which until recently led only to academic careers. In most European countries anthropological teaching has been predominantly research-oriented, that is rather (...) than ‘knowledge-oriented’ like disciplines taught in secondary education. In this paper, after having recalled some aspects of the teaching of anthropology since the 1950s, I would like to propose certain arguments for a discussion about the aims and means of teaching social sciences in general and anthropology in particular in the coming decades. Given the actual ‘dis-ease’ about this topic, this debate seems to becoming more and more inevitable. (shrink)

The number of Internet users reached 50 million in only five years compared to 13 years for television and 38 years for radio. Consumer expectation for access, convenience, and speed has made the cyberspace superhighway a medium for knowledge exchange and for e-commerce. The Internet offers a wide variety of health services and products to healthcare professionals as well as to the public. (...) Online pharmaceutical sales are predicted to reach $20–25 billion over the next four years. This is a dramatic increase when compared to the $1.9 billion in 1999. At the click of the mouse, medications can be ordered and delivered conveniently to your door. Yet, consumer discomfort with the security of information provided via the Internet is growing, and recent incidents involving disclosure of healthcare information serve to heighten this concern. The notion of a capacity in the sense of a power to bring about or undergo change plays a key role in Aristotle’s theories about the natural world. However, in Metaphysics Θ Aristotle also extends ‘ capacity ’, and the corresponding concept of ‘activity’, to cases where we want to say that something is in capacity, or in activity, such and such but not, or not directly, in virtue of being capable of initiating or undergoing change. This paper seeks to (...) clarify and confirm a certain view of how Aristotle wishes us to see the relationship between the two uses of ‘ capacity ’ and ‘activity’. Transnational Cosmopolitanism is a text that aims to build upon Kant’s account of cosmopolitanism through the post-WWI writings and political life of W.E.B. Du Bois. Through the work of these two figures, Valdez constructs the notion of “transnational cosmopolitanism” to describe situations of global injustice and to imagine worlds otherwise. By demonstrating the limits of Kantian cosmopolitanism through an anti-colonial reading of “Perpetual Peace,” Transnational Cosmopolitanism illustrates how these limits still emerge within neo-Kantian frameworks and writing. In order to overcome (...) these limitations, Valdez artfully utilizes Du Bois’ political writings and his radical coalitional politics to argue that Du Bois’ writings and actions should inform our theorization and normative insights. This text provides a unique and robust contribution to the literature of cosmopolitanism that questions the current use of Kant’s canonical approach and enlists overlooked readings and resources to compose a transnational cosmopolitanism. THIS paper is an effort to bring together two issues bearing on the moral philosophy of Hume. First, an effort will be made to interpret and clarify the role of general rules in Hume’s account of moral judgment. Second, the proper classification of the moral sentiments according to categories made familiar by studies in the philosophy of mind will be offered. The collective bearing of these two matters on the analysis of Hume’s moral theory will then be explored.

The hypothesis that perceptual mechanisms could have more representational and logical power than usually assumed is interesting and provocative, especially with regard to brain evolution. However, the importance of embodiment and grounding is exaggerated, and the implication that there is no highly abstract representation at all, and that human-like knowledge cannot be learned or represented without human bodies, is very doubtful. A machine-learning model, Latent Semantic Analysis (LSA) that closely mimics human word and passage meaning relations is offered as a (...) counterexample. (shrink)
